Below is a list of all the documents you will need to make a booking, plan your stay, and leave feedback. Use the links below to guide you to the right forms for your course. If you have trouble finding the right form please contact Duncan (01492 581218)

PLEASE NOTE: If you are downloading a form to send back to us please SAVE the document and then send it as an attachment in an email. DO NOT try to fill the form in online. Thank you.

GROUP LEADERS
To Make A Booking:-

Residential Course Booking Form - Please fill this in and send it back to us asap to confirm your booking.
Day Course Booking Form - Please fill this in and send it back to us asap to confirm your booking.
Consent, Medical and Dietary:-
Diet & Medical Summary Form - This form allows you to summarise all the medical and dietary information. It is available in an Excel document or you can produce your own if you prefer.
Adult Medical Form - This needs to be filled in by all staff on your trip and be sent in with the group's medical information.

Parental Consent Form (OSA2) - Every young person from Warwickshire must return a completed one of these to the school/organisation. Other organisations will have their own equivalent forms.    
Organising Your Trip:-
Group List - This form organises the medical information for the activity groups. It is very important that this is completed please.
Rooming List - A plan of sleeping arrangements. Please check with Duncan in the office before finalising, especially if you are sharing the centre with another group.
Essential Kit List - A kit list of everything each young person needs to bring.
Marle Hall Duties Planner - This will help you to make sure that duty groups are planned. If you are sharing you may wish to wait until arrival to fill this in.
Risk Management Summary - This document outlines our safety procedures and details of our license and insurance.
Room Layout Plan - A plan of the room locations on each floor.
Feedback
Party Leader Feedback Form - This forms an important part of our quality assurance. You will be asked to complete one during your stay. Any suggestions/comments are carefully considered and much appreciated.
Young Person Feedback Form - Feedback from the young people is just as important. Let us know how we did and what you think.
